Call our Finance Department today at (626) 576-0591 to learn what your insurance covers for physical therapy. Yonemoto Physical Therapy is contracted to accept most PPO insurance plans, Worker’s Compensation, and Medicare Insurance.
We do not accept HMOs or MediCal. However, we do accept cash and credit/debit card payment for services. We are also happy to discuss financial arrangements with you. If you are a patient paying out-of-pocket, you will need a doctor’s diagnosis for your condition and a prescription for treatment. Every insurance company is different. Depending on the insurance, you may or may not pay a co-pay and/or a deductible.
What is a Co-pay? Some insurances policies have co-pays. This requires that you pay a certain amount each visit. Co-pays are expected to be paid on your arrival of each visit. If you have questions about your co-pay, please call our Finance Department at (626) 576-0591 so we can help verify your insurance benefits. What is a Co-insurance? Some insurance policies have co-insurance. This is the percentage that your individual insurance policy requires you to pay of the allowed, or contracted, amount. We will bill you for your co-insurance amount once we determine what your carrier allows per visit. What is a Deductible? A deductible is the amount you are required to pay before your insurance begins payment. In most cases, when you see practitioners that are “in network”, you do not pay a deductible. Deductibles will vary depending on your individual policy. You can check with your insurance company to determine your individual plan coverage. If you have a deductible and have not met it, payment is expected each visit until your deductible is met.
